Population health is a new phenomenon within the health system that is defined as health outcomes of a group of individuals that includes the distribution of the highlighted outcomes within the group. The groups that are considered under population health include communities, employees within a given workplace, ethnic groups as well as other defined groups who are likely to share health outcomes. The health outcomes are developed with a significant focus on the overall need to create an improved environment under which it will be effective to ensure that there is a positive understanding on the existing elements within healthcare delivery system.
